Substance Abuse Case Manager

Position Summary

The AOD Case Manager works as a part of a multidisciplinary team of behavioral health professionals to implement the individual service plan of consumers receiving substance use disorder treatment. The AOD Case Manager shall provide care coordination, linkages to community resources, assistance in the development of recovery oriented behaviors, development of daily living skills, and assistance in developing a recovery support network.

Education

Bachelor’s Degree in human services related field.  Chemical dependency professional certification as a CDCA or higher preferred.

Experience

2-5 years work experience in a behavioral healthcare setting with a clinical focus on addiction or mental health services with adults and/ or youth.  Prior experience in case management or community psychiatric supportive treatment preferred.

Qualifications

General understanding of community based behavioral healthcare services.  Knowledge of community resources, including, vocational, housing, entitlements and general community resources utilized in providing case management.   Strong verbal and written communication skills.  Adaptable, flexible, dependable and personable.  Ability to work with a wide range of professionals within the legal field and in a correctional setting.  Requires strong organizational skills with ability to prioritize, plan and execute effectively and efficiently.   Must be able to work as part of a multidisciplinary team of professionals to develop and implement successful treatment planning and programming.  Able to interact professionally with other community agencies and provide general education/ information about agency resources and services. Must possess an Ohio Driver’s license, have access to a car and be willing to transport clients. Proficient in Microsoft Office products, and general computer skills.
