Case Manager (Criminal Justice and Mental Health)

Position Summary

The Criminal Justice Mental Health and Substance Abuse Services Case Manager is responsible for assessing the needs of persons served and working with the multidisciplinary care team to develop a community re-entry plan.  The Case Manager works to carry out community re-entry plans with clients by providing linkage with needed resources, track client progress, and develop community based interventions to work in preventing relapse, decompensation, and recidivism.  The case manager will work with adults at the Medina County Jail and youth at the Medina County Juvenile Detention Center.

Education

Bachelor’s Degree in human services related field.  Chemical dependency professional certification as a CDCA or higher.

Experience

2-5 years work experience in a behavioral healthcare setting with a clinical focus on addiction or mental health services with adults and/ or youth.  Prior experience in case management or community psychiatric supportive treatment preferred.  Experience working with the law enforcement community or within a correctional setting a plus.

Qualifications

General understanding of community based behavioral healthcare services.  Knowledge of community resources such as mental health and substance abuse treatment agencies, vocational, housing, entitlements and general community resources utilized in providing case management.   Strong verbal and written communication skills.  Adaptable, flexible, dependable and personable.  Ability to work with a wide range of professionals within the legal field and in a correctional setting.  Requires strong organizational skills with ability to prioritize, plan and execute effectively and efficiently.   Must be able to work as part of a multidisciplinary team of professionals to develop and implement successful treatment planning and programming.  Able to interact professionally with other community agencies and provide general education/ information about agency resources and services.  Proficient in Microsoft Office products, and general computer skills.
