Services

Services are available to persons age 18 and above through self referral as well as referrals by family, social service agencies, health care providers and organizations, insurance companies, etc. The agency asks that you complete an application for services which is reviewed by our admissions committee to ensure that we are the appropriate agency to meet your needs. You may chose to download or preview the application from this website. The completed form may be submitted to Alternative Paths in person, by mail, or by fax at 330.725.9187.  Staff are available to respond to any questions that you may have by phone or if you chose to come in to the agency during our usual business hours. Please do not hesitate to contact us.
